Our team is united by a shared commitment to improving health outcomes, providing exceptional service, and making a meaningful impact in the communities we serve.If you're passionate about helping others, collaborating with a purpose-driven team, and being part of a company that's transforming healthcare, we'd love to meet you.Position SummaryThe Health and Wellness Coach is responsible for developing and maintaining a wellness coaching relationship and assist patients through the process of actively working towards better health by providing support, encouragement, and education.Essential ResponsibilitiesDevelop and maintain relationships with patients who are interested in health coaching/wellness programs and provide necessary wellness coaching to reduce or eliminate high-risk behaviorsDevelop and conduct lifestyle behavioral change programs, classes, workshops, presentations, health campaigns, and other programsDevelop and distribute health education materialsTake vital signs and participate in biometric screenings by interpreting results and making appropriate referralsDevelop a customized care plan for program participants including goals and plan of actionWrite chart notes to document patient progress in both electronic medical record systems and wellness portalsAssist in developing, implementing, evaluating, and marketing wellness education events, programs, and servicesDevelop and maintain collaborative relationships with providers, nurses, fitness professionals, front desk staff, and patientsParticipate in meetings and training sessions as neededStay current on research and wellness topicsAdditional responsibilities as assignedRequired QualificationsExcellent verbal communication and public speaking skills to promote health and wellnessMust be proficient with a computer and have a working knowledge of Word, Excel, Outlook, and PowerPointMust have excellent customer service skills and problem-solving skillsAbility to work well with a team and independentlyAble to maintain client confidentialityBachelor’s degree or higher in health promotion, health education, nursing, athletic training, nutrition, other health-related fields, or counselingPreferred QualificationsRegistered Nurse or Registered Dietitian with 2-4 years of previous experience in a hospital or clinic setting is preferredTwo years’ experience as a wellness coach preferredCPR/AED certification preferred.Knowledge, Skills & AbilitiesKnowledge of specific disease and lifestyle-related topics such as weight management, nutrition, stress reduction, smoking cessation, and chronic conditionsAdequate skills in health & wellness program managementWhy Join One to One Health?Do good, help others.
