  • Ability to work independently with strong organizational and time management skills.

Responsibilities:

  • Develop, implement, and modify individualized treatment plans based on patient goals and clinical findings.
  • Provide hands-on therapy, including therapeutic exercises, manual therapy, and functional training to improve strength, balance, and mobility.
  • Educate patients and caregivers on home exercise programs, safety precautions, and use of assistive devices.
  • Document patient progress accurately and communicate effectively with physicians, nurses, and other healthcare professionals to coordinate care.
  • Manage scheduling and travel efficiently to meet patient appointment needs across various locations.
  • Stay current with best practices, clinical guidelines, and regulatory requirements related to home health physical therapy.

Skills:

The Home Health Physical Therapist utilizes clinical assessment skills daily to evaluate patient conditions and tailor treatment plans that address individual needs. Strong communication skills are essential for educating patients and caregivers, ensuring understanding and adherence to therapy regimens. Organizational and time management skills are critical for managing travel schedules and documentation requirements efficiently. Proficiency with technology, including electronic medical records, supports accurate record-keeping and coordination with healthcare teams.
