Description: The RN implements professional nursing practice centered on the patient as a whole person. The focus is on creating a caring environment that emphasizes healing and wellness. Nursing practices are based on evidence, current research, as well as department policies and procedures. Clinical decision-making incorporates all four phases of the nursing process, critical thinking, and creativity. Nursing excellence is promoted through supportive relationships among nursing staff and across other disciplines. Coordinates, ensures, and provides direct patient care activities on the unit. Responsibilities include provision of nursing rounds, assessments, and treatments. Facilitating admission, transfer, and discharge activities is also essential. The role involves providing patient and family-centered education, care coordination, and individualized treatment planning. The RN performs and documents comprehensive assessments in accordance with scope of practice and policies. Patient culture, age, and developmental level are considered during assessments. The RN identifies patient risk factors and presenting complaints. Health information is collected from patients, family, or others as necessary. Objective and subjective data are collected for analysis. Initial admission assessments are verified if obtained by other team members. A problem list is developed, prioritizing patient needs in coordination with the interdisciplinary team. Actions and interventions relevant to the patient’s culture and age are designed. Desired outcomes are determined in collaboration with the patient and family. The RN fully implements patient care therapies and procedures within established guidelines. Procedures, treatments, and processes are explained to patients and families. Actions are initiated to alleviate patient stress and discomfort. Changes in patient condition are recognized and communicated appropriately. Ongoing assessment and response to procedures and therapies are documented. Prioritization for the care of multiple patients is essential. Resources are assured and the plan of care is coordinated. Workload is organized for effective utilization of time and energy relative to patient acuity. Tasks are delegated appropriately, with oversight of team performance. The accuracy and completeness of delegated patient care are monitored. Educational needs of patients are identified and addressed relative to prioritized problems. Support for facilitating the discharge process is provided by executing discharge orders timely. Input into patient/nurse assignments is provided considering available staffing and patient acuity. Active participation in unit-based activities supporting organizational values is encouraged. A positive group dynamic and healing environment is promoted. Nursing excellence is assured in accordance with accepted standards and evidence-based practices. Participation in quality improvement activities and core measures is expected. A holistic perspective regarding patient care is utilized throughout.
