Minimum Requirements

• Doctoral Degree in Physical Therapy (DPT) from an accredited program approved by the APTA.
• Active Washington State Physical Therapy license required to start the assignment.
• CPR certification required.
• BLS certification from AHA or ARC may be required.
• Home health experience required. Experience with SOC, evaluations, ROC, recertifications, and follow-up visits required.
• Experience using Homecare Homebase (HCHB) EMR preferred.
• Ability to cover a 50-mile radius for patient visits.
• Willingness to follow a business casual dress code.
• Minimum education requirement: Doctoral.

Assignment Details

• Facility type: Home Health.
• Shift name: Days.
• Shift schedule: Days 0800-1600.
• Workweek: 5x8, 40 hours per week guarantee.
• On call/weekend requirement: No.
• Mileage reimbursed between visits from first patient to last patient.
• Coverage area: up to a 50 mile radius; candidate should be prepared to travel between patient homes within that area.
