Minimum Requirements

  • Doctoral degree in Physical Therapy (required by client).
  • Active Oregon Physical Therapy license required to start the assignment.
  • BLS certification from AHA or ARC may be required.
  • Home Health experience preferred.
  • Experience with HomeCare Homebase EMR preferred.
  • Must be able to perform PT visits including SOC, PT evaluations, recertifications, ROC, and supervisory visits.
  • Ability to cover Jackson County service area and travel between patient homes. Mileage reimbursed between visits from first patient to last patient.

  • Shift Name: Days.
  • Shift Details: Mon-Fri, 0800-1630.
  • Weekly Hours: 40 hours per week guarantee.
  • Productivity expectation: 30 points.
  • Coverage Area: Jackson County.
