Minimum Requirements

  • Bachelor‘s, Master‘s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
  • Current Physical Therapy licensure in Washington state, or an active compact license granting privilege to practice in Washington.
  • Current CPR/BLS certification (AHA or ARC accepted).
  • Valid driver‘s license, active vehicle insurance, and reliable access to a personal vehicle for travel between patient visits.
  • Experience delivering home health physical therapy services. Must be able to complete evaluations, develop and document PT plans of care, and submit progress notes per agency policy.
  • Ability to meet productivity expectations and weekly hour guarantee as required by the agency.
  • Comfort with travel across the service area, including coverage of Pierce County, WA.
  • Preferred: Experience with HomeCare HomeBase electronic documentation.

Assignment Details

  • Facility Type: Home Health.
  • Shift Name: Days.
  • Shift Details: Monday to Friday, 08:00 AM – 4:00 PM (site indicates typical schedule 8:00 AM–4:30 PM for evaluations and visits).
  • Weekly Hours: 36 (client expectation).
  • Productivity Expectation: 30 points (client guidance).
  • Coverage Area: Pierce County, WA. Candidate must travel between patient homes; mileage reimbursed between visits from first to last patient.
  • On-call/Weekend Requirement: No routine on-call; schedule flexibility and potential participation in weekend rotation or observed holiday coverage as needed per agency guidelines.
  • May be required to supervise Home Health Aides and PTAs per agency policy and state regulation.

Impacting Patient Care

Provide patient-centered home health physical therapy using evidence-based techniques. Core responsibilities include evaluating functional status, developing and implementing PT plans of care, documenting per CMS and agency requirements, communicating with physicians and interdisciplinary team members, educating patients and caregivers, and preparing discharge summaries at least every 60 days. Participate in staff conferences and in-service education as requested.
