Tufts Medical Center seeks a Registered Nurse, Home Health Case Manager to coordinate and deliver high-quality in-home care for diverse adult and geriatric patients. In this role, you will perform comprehensive assessments, develop and manage individualized care plans, provide skilled nursing interventions, and educate patients and families on disease management and medications. You’ll collaborate closely with an interdisciplinary team, ensure accurate OASIS and EMR documentation, support safe transitions of care, and champion evidence-based, patient-centered practice in a collaborative, learning-focused culture.
